Application for authorisation of elevator maintenance workshop
Notes:
The application shall be submitted to the Development Directorate of the Region concerned, which is the permanent head office of the Workshop. The relevant Development Directorate of the Region shall keep a register with the authorisations of elevator maintenance workshops. A file is kept in the register which includes: a) The licence of the elevator maintenance workshop and the original professional licences of the person in charge and the members of the mobile workshops b) The lists of the elevators maintained by this workshop in printed or electronic form c) The solemn declarations under the law. 1599/1986 for undertaking and waiving maintenance d) Any acts imposing penalties and other relevant information. The licence of the lift maintenance workshop shall be renewed every 5 years by the same authority.

The books to be taken into consideration provided for in points (a) and (b) of Article 6 of Decision F.A.9/2/EC. 28425 / 1245 (FEK 2604 B 2008), as applicable:
a. Book — Register of lifts engraved and considered by the Region Development Directorate, in whose area the seat of the conservator is in which the data of each lift, the number of stops and the type of building in which maintenance takes place or is interrupted (the Offices file number, registration number, the control body, the registration number, the full building address of the building).
b. Book — record of the maintenance engraved and deemed to be the case in which the maintenance takes place every day.

iii. that his/her auxiliary staff and the members of workshops are insured in the Social Insurance Institution or other insurance institution;
iv. the location of workshops and the telephone number - voicemail.

Description
