Solemn Declaration of the legal representative of the legal person with subject “Announcement of Recognition of a Scuba Diver Certification Body” in which it is stated that:
- He/she has not been placed in a state of bankruptcy or liquidation and no liquidator has been publicly appointed,
- He/she is not prosecuted as running away from trial or as fugitive,
- He/she has not been irrevocably convicted for the offences that constitute an obstacle to the appointment to a public servant post in Greece,
- He/she has not been placed or filed to be placed in a state of bankruptcy, privative or auxiliary court wardship,
- He/she does not exercise the profession of supplier, manufacturer, lessor or seller of diving equipment and does not participate in the share-holder structure or the management of a legal person that performs such or similar activities,
- He/she has announced the commencement of operation of the legal person in the area of the activities of the Body.

After the expiration of the above deadline, it is presumed that the profession is practiced freely (article 3 of Law 3919/2011 (Government Gazette 32 / AD / 02-3-2011). The applicant may request by the competent administrative authority (competent Port Authority) an attestation according to par. 4 ed. c of article 10 of 3230/2004 (Government Gazette 44 / AD / 11-2-2004).

Following completeness check of the supporting documents by the the Port Police Directorate, if required the legal representative of the Body sends the supporting documents asked for or provides the necessary clarifications requested, by email to the Port Police Directorate.
Notes
The legal representative may request in written the extension of the deadline for their submission up to sixty (60) days for two reasons: a) if the supporting documents submitted need - in the Service’s opinion - further investigation or clarification and b) if one or more supporting documents was not submitted in time for reasons of force majeure.
