The person concerned shall submit to the Directorate for Agricultural Economy and Veterinary Affairs (DAG) of the relevant PEI or the CEP-ECE (forms or electronics) or via the electronic CSR (forming form), the Statement of notice pre-fixed and evidence of payment of Complaints.

Description
The DAG of the company concerned, or the JEP-EKE, by submitting the responsible notification and the offender, shall issue to the person concerned a certificate of the establishment and operation of an undertaking, which is now in place for the business of the use of biocidal products for the purpose of combating insects and rodents in residential areas. In addition to the name (for a natural person) or the name of the company and its distinctive title, if any, as well as the name of its legal representative (for legal persons), with the responsibility of the person concerned, the contact address, as well as the name of the scientist in charge.
