An application for import indicating the details of the firearms, as well as the following information: (1) Surname, First Name, Fathers Name, Mothers Name, Date and place of birth, home address, ID or passport number, date of issue and issuing authority, occupation, telephone number and email. It must be expressly stated that it concerns a merchant.
