Certificate from a competent authority of the Member State of establishment that the provider is legally established therein and is lawfully pursuing that professional activity and that he has not been prohibited, even temporarily, from exercising it at the time of issue of the certificate.
If no authorization is provided for the specific activity by the Member State of establishment, a certificate from the competent authority of the Member State of establishment regarding the legal incorporation of the company and its legal representative shall be provided.

Description
After the completion of internal administrative audits regarding the application and the submitted documents, the Audit Committee of the relevant Regional Authority Unit carries out the on-site inspection of the professional space to be established.
Notes
-In case the Audit Report has findings, an executive of the Department of Private Primary Health Care Units prepares a draft document with the findings of the Audit Report and signs it to inform the interested party. At this point in the procedure, the person concerned does not have the right to operate the aesthetics laboratory and to obtain this right he must re-submit all the supporting documents.
-In case the Audit Report does not record findings, an executive of the Department of Private Primary Health Care Units prepares a draft Certificate of Commencement of Operation and sends it to the hierarchy for signature.
